Most of our teams have been selected for the 2010 season, but if you are still interested in playing hockey, please contact us and we can discuss your situation and place you on a waiting list. The only age group we would like more players for at this stage is our Under 11 team, so if you have any friends or family you think would like to learn to play, please contact us.
Registration forms are available to download by choosing either Junior or Senior Registration Fees from the product categories in the shopping menu on the left hand side of the page, choosing what type of hockey you will be playing, and completing the payment process.
Payment for your registration fees using this website will be made by PayPal. Don't worry if you don't have a PayPal account - if you have a credit card, you don't need to! Simply choose the items you want, complete the checkout process and input your details as required into PayPal's secure payment form. When payment is confirmed, you will receive an email with a link to download either the Junior or Senior Registration Form. Please complete BOTH pages and return to the club with a printed copy of your PayPal payment receipt (and birth certificate and/or clearance notice if applicable), by any of the methods listed in that email.
Alternatively, our other payment methods (cheque, credit card, cash or money order (made out to South West United Women's Hockey Club Inc.) are available. Please contact us if you would like to enquire about this. You can download the forms by choosing either Junior or Senior Registration forms from the links below. This includes the BWHA Membership Form that you will also need to complete. Please remember that full payment is required when lodging your forms (fees are noted on the forms). Don't forget to send in a copy of your birth certificate and/or clearance notice, if applicable.

All players must pay their full year's fees at the time of registration. Unfortunately we cannot accept any registration without full payment. In certain circumstances, a payment arrangement may be agreed with the Treasurer.
All players and parents also need to read and abide by our club's Codes Of Conduct and Member Protection Policy (available from the Codes Of Conduct page).
PLEASE NOTE: Birth Certificates and Clearance Notices
All Junior and Masters players who are NEW to the club must provide a copy of their birth certificates (which we need to retain).
If you have previously played with another club, (within the last 3 years) you will need to obtain a “Clearance” from that club and send a copy of this with your forms. A “Clearance” is a letter stating that you have fulfilled all your obligations with that club.
